The links will take the member to Acrobat.com- you must download the form to fill it in electronically.

OKCMAR allows for electronic signature, using SoftShare, for your applications. When submitting the forms a Soft Share dialogue box will appear. (Brokers please note that your foms rmay require mutliple digital signatures- each time you are asked to replace the existing file click OK) The following information will appear:

1. Sign As: This is a drop down box - choose NEW ID
2. Choose: Pick A NEW DIGITAL ID I WANT TO CREATE NOW from the radio button list of choices
3. Choose: Either the Windows Certificate Store or NEW DIGITAL ID FILE
4. File Location: This will autofill - make sure all identity fields are complete
5. Create & Confirm a password- then click finish. The application will now display a digital signature and is ready to submit
