If you have any questions about which form to use please call membership at 405-840-1493.
Clicking on the form will bring up adobe acrobat. The program will ask to "Download the File". Download the file into either "My Documents" or "My Desktop". Once the file has been saved in your computer-click on it to open, fill out and submit with your digital signature.
A Tip: Once you have saved the files that you need go to "My Recent Documents" on your start panel- they will all be listed there.
